I hereby acknowledge and agree that ISTC has a legitimate business need to verify my identity before I complete the training and safety course. Therefore, I authorize and instruct ISTC and S2Verify, a consumer reporting agency, that it may obtain, assemble, and use any information, including a social security number consumer report, to help verify my identity based upon the identifiers and/or social security number that I am providing below. I hereby give permission to any source that may have these records to release such records to ISTC and S2Verify. I also agree that a fax or photocopy of this authorization with my signature be accepted with the same authority as the original. I understand that I have the right to make a request of S2Verify upon proper identification and the payment of any legally permissible fees, for the information in its files on me at the time of your request. By continuing to use the LMS, I hereby acknowledge that I have read and understand the above and instruct ISTC and S2Verify to obtain, provide, and use a consumer report to help verify my identity.
